Lost/Stolen Passports
The loss of your passport while traveling abroad is a serious matter but it is one that the Consulate can help you resolve. Please follow the steps outlined below to report the loss of your passport and obtain a replacement.
1. Lost or stolen passports should be immediately reported to the local Public Security Bureau. They will issue an original police report which you will need in order to obtain a replacement passport and visa. You should take a copy or your passport or another form of ID with you when you go the Public Security Bureau.

You should also report the loss to the Consulate by telephone at (028) 8558-3992 or email at ConsularChengdu@state.gov if you do not plan to obtain a replacement passport immediately.
2. To apply for a replacement passport you should come to the Consulate in person during regular American Citizen Services hours. Please bring the following items:
2 passport-sized photos
Original police report from the Public Security Bureau
Passport fee
Any proof of identity you may have (e.g. drivers' license or student ID with photo)
Any proof of U.S. citizenship you may have (e.g. birth certificate, canceled US passport, photocopy of your passport information page)
You can download and print the required DS-11 and DS-64 forms, or you may fill out the forms at the Consulate.

Under ordinary circumstances the Consulate issues travelers a regular, full validity passport to replace the lost passport. These passports are printed in the U.S. and take 7-10 days to arrive in Chengdu. Depending on the circumstances the Consulate may issue limited validity, emergency passports on a case by case basis. Emergency passports may be issued while you wait and have a validity of no more than one year.
4. Once you have obtained a replacement passport you should promptly contact the Chinese Exit and Entry Department to obtain a temporary travel visa. Failure to obtain a replacement visa before attempting to leave China can result in a fine and delays. You will need the police report from the Public Security Bureau to obtain a replacement visa. The fee for a travel visa is approximately RMB160 and the standard processing time is five days.
